know that you can get your PAS (Payment
Authorization Statement) from
the state's web site?
Click here to go to the state's
website, and sign on with your
user ID and Password.
Two courses are currently available and
they include:
1) Major Unusual Incidents (MUI)
training which is required by OAC
chapter 5123:2-17-02 for all County
Board employees and providers on an
annual basis.
AND
2) Individual Rights training which is
required to be completed by providers
who wish to be certified to provide
Level One Waiver services.

Supported Living
providers who do not yet have their
Waiver Certification should call Chuck
Smith, ODMRDD at (614) 995-7080, for an
application. Opportunities for renewal
of or for new SL contracts are
diminishing while IO Waiver and Level 1
Waiver contract opportunities are
expanding in the coming months.
